Use the form on this page to update us on activities you’ve completed within a quarter. Each quarter’s dates are as follows:

First quarter: January through March; final reports due April 15.

Second quarter: April through June; final reports due July 15.

Third quarter: July through September; final reports due October 15.

Fourth quarter: October through December; final reports due January 15.
